We are excited to offer Full-Time
positions for Level 3 Community Support Worker roles for our Housing & Accommodation Support Initiative (HASI) Program in the
South Eastern Sydney area. Usual hours of work vary from Monday to Sunday between 6am to
8pm with a rotating roster.

This position supports the delivery of community-based support in the Mental
health space.
The Day to Day
Providing support to Participants in the mental health community outreach
services, to improve their independence by assisting with living skills,
transport, social activities, medication runs as well as other supports as
required.
Keeping customer records up to date using online systems.
Developing and maintaining professional relationships with people, families,
caregivers and key stakeholders.
Complete risk assessments, support plans and other required documents in a
timely manner.

Invested in providing the highest quality in Mental Health services to the
community.
Competent in working with people living with Mental Health.
What you need to have
Qualification in Mental Health or Community Services, OR equivalent relevant
experience in the human services industry.
Experience in the mental health and community sector, with strong knowledge of
services, block funded Mental health programs, NDIS and Understanding of
Person-Centered Practices.
Current Police check – not older than 3 months
Valid NSW Drivers C class licence, with ability to drive vehicles in licence
class.
Valid first aid certificate.
NDIS Workers Check
NSW WWCC
Basic skills in Microsoft Office, other common systems.
